Cancellation Policy. If you have made payment and need to withdraw from the course you have registered for:
1. More than two weeks before the starting date of the course, you may either transfer to a course on a later date or forfeit 15% of the amount paid.
2. Within two weeks of the starting date of the course, you may either transfer to a course on a later date or forfeit 50% of the amount paid.

I agree to abide by the following conditions when completing my clinical hours at the allocated care home / retirement facility:

• To arrive on time every time and not to leave early.
• To have my hours signed off each day that I attend the facility.
• To arrive in uniform, as prescribed by your instructor, for each shift.
• To wear my hair & uniform neatly and with pride.
• No smoking prior to or during my shift or class.
• No replacing myself with someone else; only I can complete my hours.
• I will not cancel any shift 
• I will not hold Survival Care Giver Training (Pty) Ltd. accountable for any loss, death, or injury incurred whilst attending classes or completing my clinical hours.
• No consumption of alcohol during class or during shift.
• I will not use my cellphone while busy with a patient. My cellphone will be locked away except when on a break.
• If any of the above conditions are not met, you will not be able to graduate.
• If you are dismissed or asked to leave a care home for any reason, we will not find an alternative care home for you, and your certificate will state that your clinical hours are incomplete.
