* No module may be missed unless due to severe circumstances. If a module is missed, you will not be allowed to complete the course or graduate. Should you experience a severe circumstance, please notify your instructor timeously so that an alternative arrangement can be made for you for the module.
* I hereby confirm that I will complete the full 9-week Carer Course and will pay the full course fee for the Survival Carer Course Program before I graduate.
* I hereby confirm that I understand the payment plan for the 9-week Care Giver Course for 2026 is R3,000 before the course starts, R2,700 in week 3 of the course and R2,700 in week 6 of the course.
* I hereby confirm that I understand that no payment plan is offered for the 9 Day Care Giver Course and that the full course Fee needs to be paid before the start date of the 9 Day Care Giver Course
Cancellation Policy
* If you have made payment and need to withdraw from the course you have registered for:
1. More than two weeks before the starting date of the course, you may either transfer to a course on a later date or forfeit 15% of the amount paid.
2. Within two weeks of the starting date of the course, you may either transfer to a course on a later date or forfeit 50% of the amount paid.
3. In the event that a student withdraws from the course after commencement, they shall remain fully liable for the full course fee, including any instalments not yet paid, as of the date of written notice of cancellation.
* No refund, full or partial, will be granted for early withdrawal, regardless of attendance or progress made.
* I agree to abide by the following conditions when completing my clinical hours at the allocated care home/retirement facility:
a) To arrive on time every time and not to leave early.
b) To have my hours signed off each day that I attend the facility.
c) To arrive in uniform, as prescribed by your instructor, for each shift.
d) To wear my hair and uniform neatly and with pride.
e) No smoking prior to or during my shift or class.
f) I may not substitute or replace myself with another individual; only I can complete my hours.
g) I will not cancel any shift.
h) I hereby indemnify Survival Care Giver Training (Pty) Ltd. against being accountable for any loss, death, or injury incurred whilst attending classes or completing my clinical hours.
i) No consumption of alcohol during class or during shift.
j) I will not use my cell phone while busy with a patient. My cell phone will be locked away except when on a break.
k) Should any of the above conditions not be met, you will not be permitted to graduate.
l) If you are dismissed or asked to leave a care home for any reason, we will not find an alternative care home for you, and your certificate will state that your clinical hours are incomplete.
